Introduction:
PABC is diagnosed during pregnancy or in the first postpartum year. It is the second most common malignancy affecting pregnancy is breast cancer which affects 1 in 3000. The average age of women with PABC is 32 to 38 years. Only 6.5% of all cases of breast cancer affect women age < 40 years. As more women are delaying childbearing, and as breast cancer rates continue to rise, more diagnoses of PABC are anticipated. However, because PABC is a relatively rare event surrounded by multiple variables, few studies address the best management and treatment options.

Case presentation:
She presented with 3 months duration of Right breast lump with 13 weeks of pregnancy. She Has 3 children aged 10,5 and 3 yrs. Significant family history of paternal aunt with breast cancer at 40, Paternal grandmother at 70, Paternal great aunt BC and maternal great aunt with ovarian cancer. Clinically there was a 30 mm lump in the right breast with indentation of skin and normal axilla. USS showed 29 mm U5 lump. Mammo not done due to pregnancy. 20 weeks scan normal. Biopsy showed 28 mm Grade 3 IDC .ER 7 and HER2 neg. Estimated delivery 0ct 2022 Treatment She had Mastectomy with SLNB, and 2/3 nodes were involved. At 20 weeks Onco review done and started on adjuvant chemo 2nd-3rd trimester 5 cycles and stopped. Had LSCS and Right ANC done showed a Total 6/14 positive nodes. She had Adjuvant chemo with Abemaciclib and Zometa , followed by RT chest wallSCF/IMC ,Tamoxifen and zoladex.
