Abstract:
An effective nursing model of care is vital in any institution to achieve overarching goals of the nursing department and to help guide a nurse’s scope of care. The foundation of this work was created around the organizational primary value of “The needs of the patient come first” as well as the core values of respect, integrity, caring, healing, teamwork, innovation, excellence, and stewardship. Despite having just opened in 2019, the nursing department at Sheikh Shakhbout Medical City (SSMC) wanted to outline the nursing work that was important to staff and patients on the journey to excellence.  The SSMC Nursing Model of Care framework is based on four core elements referred to as pillars.  First, Person-Centered Care which respects individual values and choices as well as promoting continuity of care. Second, is the Team-Based Care which fosters teamwork and provides a supportive work environment.  Workplace culture is important to promote professional excellence and career development. Third, is Trusted Quality Care focused on providing our patients and their families with safe, efficient, and effective care. Finally, Value Added Care promotes optimal and continuous flow throughout the patient’s journey and reduces inefficiencies and waste. This framework includes 27 initiatives aligned with the four pillars.  Each initiative has quantifiable measures which are designed to achieve the model’s overall success.  Success is measured on four dimensions: improved patient and staff satisfaction, clinical outcomes and operational efficiency.
