GROUP WORK EXPERIENCE IN CREATING A PATIENT SAFETY CULTURE WITH CREATIVE DRAMA TECHNIQUE: A MIXED METHOD STUDY

Abstract:
The research was carried out in order to describe the views, thoughts and knowledge of nurses about patient safety by determining the patient safety culture, to create a patient safety culture with the patient safety precautions training given with the interactive theater technique, and to reveal the effect of the patient safety culture on the formation of the patient safety culture by basing it on qualitative and quantitative data in depth. It is a mixed method study in an experimental exploratory sequential design.
The research was carried out in a private hospital in Izmir between November 2019 and June 2020.“The sample of the study was determined by the criterion sampling method and the study was completed with a total of 23 nurses who scored lower than the hospital patient safety culture score (below 60 points). In the collection of data; “Nurse Introduction Form”, “Patient Safety Culture Hospital Questionnaire”, “Patient Safety Culture Semi-Structured Interview Form” and “Post-Training Interview Form” were used. The process of the research structured within the scope of the mixed method; It is discussed in five main sections as collecting preliminary data, identifying participants, providing face-to-face interviews, providing trainings and collecting final data. SPSS 15.0 package program was used in the evaluation of quantitative data analysis. Number percentage distribution and descriptive statistics were used to define demographic characteristics, chi-square test was used to evaluate the homogeneity of the groups. Since the sample number was less than 30, Mann Whitney U test, Wilcoxon Signed Ranks test, and Friedman analysis of variance were used for repeated measurements. The MAXQDA 20 program was used to obtain the content analysis of the qualitative data. The audio recordings obtained were deciphered and the evaluation reports were analyzed with thematic content analysis method. Codes and sub-codes were created on the program and the visual tools of the MAXQDA 20 program were used in the preparation of the visual content of the analysis. Written permission was obtained from the Ege University Faculty of Medicine Clinical Research Ethics Committee and the private hospital where the study was conducted, in order to conduct the study. Informed consent and verbal consent were obtained from individuals who agreed to participate in the study and who complied with the limitations of the study. Of the nurses participating in the study, 39.1% worked in the intensive care unit, 65.2% were clinical nurses, 78.3% were 27 years and younger, 87% were female, 60.9% were high school graduates, and 65% were It was determined that, 2% of them had less than 6 years of working years, 87% of them received training on patient safety precautions, and 87% of them did not create an error event reporting process. In the comparison of the nurses in the experimental and control groups, there was a significant difference between the post-test average scores of the sub-dimensions of teamwork, staffing and non-punitive response to mistakes (p<0.05), and changes in patient safety culture over time with patient safety culture trainings. It was determined that there was an increase in the mean scores of the nurses in the experimental group before and two months after the education, and that there was no positive change in the nurses in the control group, and there was no statistically significant difference in the comparisons between and within the groups (p>0.05). It has been determined that there is an increase in the number of re- ported events as the number of events reported in the units where nurses work changes over time. As a result of the qualitative data analysis of the research, 3 main themes, 10 sub-themes and 54 sub-dimensions were determined and discussed with three main themes as “patient safety definitions”, “medical error” and “error reporting”. The results of the change in nurses’ thoughts on event reporting over time, the obstacles that threaten patient safety, the attitude of the employee and the manager about error reporting are among the results that support each other in qualitative and quantitative findings. It has been suggested that creative drama technique can be used in team communication, manager expectations and activities to improve patient safety, development of error reporting processes and conveying their importance to employees, measuring the level of knowledge on patient safety measures in studies to be carried out on patient safety, increasing the sample size and conducting research in different professions.

Ms. Sinem Eroğlu completed her doctorate degree at Ege University Health Sciences Institute in 2021. Following her undergraduate education, she worked as a training nurse, infection control nurse and nursing services training coordinator in various hospitals. He worked as a lecturer and mentor at universities. Eroğlu, who specializes in areas such as patient safety, nursing education, time management, creative drama and NLP, has various articles. He has worked on innovative ideas. He made oral and poster presentations at many congresses and symposiums. He was deemed worthy of the “Oral Presentation Second Prize” with the oral presentation he presented in 2009.
