First Experience with Endoscopic Intraluminal Gastroplasty Using the Apollo Overstitch Stapling Device.

Relevance: Given the rising obesity epidemic and rising healthcare costs, the need for less invasive and cost-effective bariatric treatment methods is becoming clear. Bariatric surgery has demonstrated high efficacy not only in weight loss but also in achieving remission from type 2 diabetes. Despite success, complications after bariatric surgery can reach up to 29% (Steenackers N, 2023). The advent of new devices that allows gastric volume reduction without incisions or scars and achieving the desired weight loss, has expanded the scope of care for overweight patients.

Study objective: Short summary of the initial experience of endoscopic endoluminal gastroplasty in patients with stage 2-3 obesity.

Materials and methods: Between February and July 2025, 7 patients (3 men and 4 women) underwent endoscopic sleeve gastroplasty at the JAMSI Medical Center. Patients ages ranged from 30 to 47 years with a mean age of 42 years. The patients’ body mass index ranged from 33.8 to 48.3 kg/m². All patients underwent gastroplasty using a dual-channel gastroscope (Olympus) and an Apollo-Overstitch dual-channel gastroscope system. The procedures were performed in the operating room under endotracheal anesthesia. The patients were positioned in a supine position on the operating table, with the table rotating up to 20 degrees to the left. Esophagogastroduodenoscopy was performed prior to surgeries to rule out mucosal pathology. An Overtube splint was used to safely insert the endoscope with the Apollo-Overstitch system. CO2 was used for insufflation during the endoscopic intraluminal surgeries.

Outcomes: The greater curvature was closed from the anterior to the posterior wall, with a transition to the anterior wall (U-shaped suture) in six patients. One of the patients underwent Z-shaped suture, from the anterior to the posterior wall. All patients received four sutures for volume reduction, which resulted in a 50-70% reduction in the total gastric volume. The surgical procedure lasted from 75 to 150 minutes. No complications related to the surgical technique were observed. One  of the patients did a control abdominal X-ray which revealed free gas (carboxyperitoneum) in the early postoperative period (1 day), without clinical manifestations, which did not require additional treatment. During the first three months, patients reported weight loss of approximately 5 to 33 kg, with the first 5-10 kg loss observed within the first 10 days.

The main complaint of patients in the postoperative period was nausea, which was relieved by metoclopramide. All patients were discharged within 1-2 days after surgery.

Conclusion:

The technique of endoscopic gastroplasty with endoluminal suturing using the Apollo-Overstitch suturing system is a new, minimally invasive technique in bariatric surgery. Our study demonstrates the safety and feasibility of this technique in patients with morbid obesity, which requires further research and increased experience.
