Introduction:  A sudden unexpected death can be found in all circumstances. Death can occur when people are exercising, active or resting [1]. Sudden death itself is not always a sudden process. Sudden death is the end of a disease whose victims have died suddenly. Sudden death caused by the disease often raises suspicion for investigators and the general public, especially if the death happened to someone who is known in the community and found in a public place [2].

A body was found in the residential area on Jalan Ngagel 165 Surabaya, on Monday, January 10, 2022, at 11.45 West Indonesian Time and reported to the police for investigation. A forensic expert identified the body at Dr Soetomo General Hospital. External, internal, and laboratory investigations were conducted. An open wound was found on the left breast measuring thirteen centimeters long, twelve centimeters wide, eight centimeters high, and five centimeters deep, accompanied by a white, pus-like fluid on external examination with signs of asphyxia. In an internal examination, there were bleeding spots on the cerebrum, cerebellum and brain stem. From the respiratory tract to the branching of the lungs found, a white fluid similar to pus. Both lungs found adhesions with the chest and back walls. The spine is not symmetrical to the fourth through twelfth vertebrae. On anatomical pathology examination, lung samples were found to have pulmonary oedema; and the breast samples showed invasive breast carcinoma of Non specified type (Infiltrating Ductal Carcinoma) NOS Grade I.

Forensic autopsies are carried out due to an unnatural cause such as accidents, homicide or suicide. This examination aims to help determine the identity of the corpse, determine the exact cause of death, the mechanism of death, and the time of death[3] [4].
