Evaluation of Argon Plasma Coagulation-Based Anti-Reflux Mucosectomy and Ablation (ARMA) Technique for Treatment of Gastroesophageal Reflux: A Case Series

Background: Gastroesophageal reflux disease (GERD) remains a common and chronic condition, often requiring long-term pharmacologic management. Endoscopic approaches such as Anti-Reflux Mucosectomy and Ablation (ARMA) using Argon Plasma Coagulation (APC) have emerged as minimally invasive alternatives.

Methods: We performed the ARMA technique on 30 patients diagnosed with GERD, including 28 who underwent standard ARMA and 2 who received an extended version involving additional band ligation. The primary objectives were to evaluate symptom improvement and reduction in the need for anti-reflux medications. Clinical outcomes were assessed using a Visual Analog Scale (VAS) and patient-reported symptom relief. Patients were followed for a mean period of approximately three months.

Results: Complete clinical response—with minimal need for medication—was observed in approximately 20% of patients. A partial response with improved symptom control, reduced medication dosage, and improved quality of life, especially nocturnal reflux relief, was noted in 53.3% of cases. The remaining 26.6% of patients reported no significant symptomatic improvement. The procedure was generally safe and well-tolerated. No major complications such as bleeding or dysphagia were reported. Five patients experienced transient retrosternal discomfort radiating to the back for 24–48 hours post-procedure. The addition of band ligation increased procedure time and cost but did not result in enhanced clinical outcomes compared to the standard ARMA. Notably, male patients showed a slightly better clinical response than females.

Conclusion: ARMA appears to be a safe and effective endoscopic technique for managing GERD in selected patients, offering meaningful symptom relief and medication reduction in a majority of cases. Further prospective studies with longer follow-up are warranted.
