Comprehensive Approach to Strengthening Endoscopic Healthcare Infrastructure with the mikoto Simulator

Introduction: The 5-year survival rates for gastrointestinal (GI) cancers vary widely worldwide. Japan reports 75% for gastric cancer and 76% for colorectal cancer, among the highest globally, yet a substantial gap persists elsewhere. A major determinant of this disparity is the availability of endoscopic screening systems. Successful screening requires a sufficient number of endoscopists and trained technicians. Japan maintains approximately 28 endoscopists per 100,000 population, whereas many countries have only one-tenth to one-thirtieth of that density.

Endoscopic techniques are diverse and demand considerable time and financial resources for training. To address this challenge, we developed the mikoto endoscopy simulator, which enables efficient, self-directed learning and provides the world’s first objective assessment of endoscopic technical skills. Our initiatives include global dissemination of the mikoto simulator, creation of a structured endoscopy training curriculum, establishment of a non-profit organization to certify endoscopic skills and education, and development of an integrated system to aggregate training data generated by the simulator.

Through these comprehensive efforts, we aim to elevate the global endoscopic healthcare infrastructure to the current Japanese standard within the next decade, thereby reducing preventable mortality from gastrointestinal cancers.

Biography: Masashi Fujii, MD, PhD, earned his doctorate in 2019 at the Next-Generation Advanced Medical Center of Tottori University Hospital, focusing on medical device development. A board-certified gastroenterologist, he has extensive experience in endoscopic diagnosis and therapy while serving as Chief Medical Officer for a start-up developing medical simulators. In 2020 he founded R ZERO Inc., a Tottori University spin-off, and as CEO established the first private clinical laboratory in the San’in region to provide PCR testing for COVID-19 and organized large-scale vaccination centers in collaboration with local governments. In 2022 he opened Kurayoshi City Endoscopy Clinic to deliver community-based cancer screening and continues research and doctoral supervision on medical device innovation at Tottori University, where he is an Assistant Professor, and a part-time lecturer at Tottori Junior College. In 2025 he founded and became Chair of the International Endoscopic Preventive Medicine Foundation and is currently pursuing a doctoral program in Information Engineering.
