A Tale of One Academic Transfusion Service in Response to Blood Shortage Around COVID19 Pandemic

Biography: Fatima Aldarweesh, MD, is an Assistant Professor and specializes in Blood Bank and Transfusion Medicine. She received her MD degree from King Faisal, University, Saudi Arabia, and completed residency training at Brown University and fellowship training at University of Wisconsin.

Introduction:
The first few weeks of the COVID-19 pandemic caused a massive disruption, with contraction of the blood supply by 143,600 units
1. Infectious spikes with the omicron and delta variants, and mandated postinfection quarantine continued the blood shortages
2. There was an estimated 10 % contraction of the blood supply
3. Although blood centers supply nationally, the experience was highly region-specific. Illinois underwent some of the most severe COVID-19 restrictions from 2020 to 2022. This resulted in critical blood supplies during the initial 3 months, for the duration of the omicron and delta surges, and the holiday seasons.

Study design:
Blood utilization data from a trauma level 1 medical center; the University of Chicago was compiled around the different pandemic periods. Several strategies such as prospective auditing for platelets and cryoprecipitate, EHR banners and published information on the intranet. Additional strategies were deployed such as a mass notification system, daily inventory emails, remote blood storage inventory restrictions, cultivation of in-house blood donation, and an institutional blood shortage management plan.

Conclusion: Although the blood shortage experience is variable, exercising best transfusion practices, blood conservation and stewardship remained the most important factors.
